RESUMO

Objective:To observe the characteristics of choroidal thickness in patients with macular edema secondary to superior temporal branch retinal vein occlusion (BRVO-ME).Methods:A retrospective control study. From November 2020 to September 2021, 30 patients (30 eyes) with BRVO-ME (BRVO-ME group) were diagnosed by ophthalmology examination in Department of Ophthalmology, The Affiliated Hospital of Chengde Medical College and 14 healthy volunteers (28 eyes) were enrolled in the study. The choroidal thickness of macular area was measured by enhanced deep imaging technique of frequency domain optical coherence tomography. According to the subdivision of the diabetic retinopathy treatment group, the choroid within the 6 mm of the macular fovea was divided into three concentric circles with the macular fovea as the center, namely, the central area with the diameter of 1 mm, the inner ring of 1-3 mm and the outer ring of 3-6 mm. The inner ring area and the outer ring area are divided into upper, lower, nasal and temporal sides, respectively, which are denoted as S3, I3, N3, T3 and S6, I6, N6, T6, totaling 9 areas. To observe the distribution characteristics of choroidal thickness in different regions of two groups of eyes. The choroidal thickness of different macular regions was compared by independent sample t-test. Results:The choroidal thicknesses in the central area, S3, T3, I3, N3, S6, T6, I6, and N6 of the eyes in the control group and BRVO-ME group were 214.11±56.04, 207.89±57.92, 214.07±54.82, 207.14±61.54, 180.18±53.53, 204.25±59.60, 193.93±51.50, 190.54±51.21, 139.82±39.84 μm and 258.00±71.14, 256.43±68.70, 252.07±72.97, 244.37±68.49, 243.10±70.93, 247.20±68.36, 221.00±61.28, 223.77±58.64, 183.20±60.15 μm. In both groups, the choroidal thickness was the thickest in the central area, gradually thinning to the nasal side and temporal side, and the nasal choroidal thickness was thinner than other regions, and N6 area was the thinnest. Compared with the control group, the choroidal thickness of central area, S3, T3, I3, N3, S6, I6 and N6 in BRVO-ME group were significantly thicker ( t=-2.899, -2.229, -2.172,-3.250, -2.543, -2.292, -3.214; P<0.05), there was no significant difference in T6 area ( t=-1.814, P=0.075). Conclusion:The choroidal thickness of macular area in patients with BRVO-ME is thicker than that in normal subjects.

Objective:To construct a surface display system containing various lengths of the Ag43 passenger domain for an optimal bacterial surface display of foreign protein HPV16L1.Methods:(1) Ag43 gene sequences of different lengths were inserted into pET22b vector to construct four Ag43 surface display vectors (Ag43/138, Ag43/551, Ag43/552 and Ag43/700) using PCR and subcloning strategy. (2) The generation of four HPV16L1-Ag43 fusion constructs was completed by PCR and subcloning methods. (3) HPV16L1-Ag43 fusion proteins were expressed and analyzed by SDS-PAGE. (4) The surface exposure of HPV-16L1 was verified using trypsin digestion.Results:PCR analysis and sequencing results showed that Ag43 surface display vectors and HPV16L1-Ag43 fusions were constructed successfully. SDS-PAGE showed that the expression of HPV16L1-Ag43 fusion proteins could be induced with 0.2 mmol/L IPTG and the protein content was reduced after the cells were treated with trypsin, especially the content of Ag43/700-HPV16L1 that showed a drastic reduction.Conclusions:The Ag43 surface display system was successfully constructed and could be used for a successful display of HPV16L1. This study also showed that Ag43/700 comprising only the α-helix and the β-barrel of Ag43 provided an optimal surface display for HPV16L1.

Objective@#To retrospectively analyze the distribution of non-fermentative bacteria causing bloodstream infection in hospitalized patients in Sichuan Province and their drug susceptibility to common antibiotics for better understanding their epidemiological characteristics.@*Methods@#From January 1, 2015 to December 31, 2017, all of the non-fermentative bacteria isolated from patients with bloodstream infection in nine hospitals in Sichuan Province were collected. Species distribution and drug resistance test results were retrospectively analyzed.@*Results@#A total of 6 291 strains of pathogenic bacteria were isolated, including 3 674 strains of gram-negative bacteria (58.4%) and 2 617 strains of gram-positive bacteria (41.6%). The gram-positive bacteria were 1 895 strains of Staphylococcus (30.1%), 372 strains of Streptococcus (5.9%), 317 strains of Enterococcus (5.1%) and 33 strains of other gram-positive bacteria (0.5%). The gram-negative bacteria were 3 191 Enterobacteriaceae strains (50.7%), 389 non-fermentative strains (6.2%) and 94 other gram-negative strains (1.5%). The isolated non-fermentative bacteria were mainly Pseudomonas aeruginosa (136 strains, 35.0%), Acinetobacter baumannii (126 strains, 32.0%) and Stenotrophomonas maltophilia (33 strains, 8.5%). There were 167 (42.9%), 112 (28.8%) and 82 (21.1%) non-fermentative bacteria isolated in internal medicine departments, ICUs and surgery departments, respectively. The drug resistance rates of Pseudomonas aeruginosa to cefepime, ciprofloxacin and gentamicin were 16.5%, 10.7% and 9.9%, respectively. Carbapenems-resistant Pseudomonas aeruginosa (CR-PA) accounted for 13.6%. No polymyxin-resistant Pseudomonas aeruginosa was found. The resistance rates of Acinetobacter baumannii to all antibiotics were over 30% except for minocycline and polymyxin and 75.7% of carbapenem-resistant Acinetobacter baumannii (CR-AB) were isolated. The drug resistance rates to levofloxacin, trimethoprim/sulfamethoxazole and ceftazidime were 0%, 0% and 37.0% in Stenotrophomonas maltophilia isolates and 10.5%, 4.2% and 19% in Burkholderia cepacia isolates, respectively.@*Conclusions@#Non-fermentative bacteria causing 6.2% of bloodstream infection in Sichuan, mainly by Pseudomonas aeruginosa and Acinetobacter baumannii. There were significant differences in the isolation rate of non-fermentative bacteria in different departments and most of the strains were isolated in internal medicine departments. The detection rate of Acinetobacter baumannii with multiple drug resistance was high, while other non-fermentative bacteria had good sensitivity to antibiotics.

Objective To retrospectively analyze the distribution of non-fermentative bacteria cau-sing bloodstream infection in hospitalized patients in Sichuan Province and their drug susceptibility to com-mon antibiotics for better understanding their epidemiological characteristics. Methods From January 1, 2015 to December 31, 2017, all of the non-fermentative bacteria isolated from patients with bloodstream in-fection in nine hospitals in Sichuan Province were collected. Results A total of 6291 strains of pathogenic bacteria were isolated, including 3674 strains of gram-negative bacteria ( 58. 4%) and 2617 strains of gram-positive bacteria (41. 6%). The gram-positive bacteria were 1895 strains of Staphylococcus (30. 1%), 372 strains of Strep-tococcus (5. 9%), 317 strains of Enterococcus (5. 1%) and 33 strains of other gram-positive bacteria (0. 5%). The gram-negative bacteria were 3191 Enterobacteriaceae strains (50. 7%), 389 non-fermenta-tive strains (6. 2%) and 94 other gram-negative strains (1. 5%). The isolated non-fermentative bacteria were mainly Pseudomonas aeruginosa ( 136 strains, 35. 0%), Acinetobacter baumannii ( 126 strains, 32. 0%) and Stenotrophomonas maltophilia ( 33 strains, 8. 5%). There were 167 ( 42. 9%), 112 (28. 8%) and 82 (21. 1%) non-fermentative bacteria isolated in internal medicine departments, ICUs and surgery departments, respectively. The drug resistance rates of Pseudomonas aeruginosa to cefepime, cipro-floxacin and gentamicin were 16. 5%, 10. 7% and 9. 9%, respectively. The resistance rates of Acinetobacter baumannii to all antibiotics were over 30% except for minocy-cline and polymyxin and 75. 7% of carbapenem-resistant Acinetobacter baumannii ( CR-AB) were isolated. The drug resistance rates to levofloxacin, trimethoprim/sulfamethoxazole and ceftazidime were 0%, 0% and 37. 0% in Stenotrophomonas maltophilia isolates and 10. 5%, 4. 2% and 19% in Burkholderia cepacia iso-lates, respectively. Conclusions Non-fermentative bacteria causing 6. 2% of bloodstream infection in Si-chuan, mainly by Pseudomonas aeruginosa and Acinetobacter baumannii. There were significant differences in the isolation rate of non-fermentative bacteria in different departments and most of the strains were isolated in internal medicine departments. The detection rate of Acinetobacter baumannii with multiple drug resistance was high, while other non-fermentative bacteria had good sensitivity to antibiotics.

Objective:To set up the rat skeletal muscle L6 cell models of oxygen-glucose deprivation(OGD)in vitro,and to investigate the protective effect of EGF in deep tissue inj ury(DTI)of pressure sores.Methods:The rat skeletal muscle cells in the logarithmic phase were divided into normal control group,OGD group,5 μg·L-1 EGF+OGD group,10 μg·L-1EGF+ OGD group and 20 μg·L-1EGF+ OGD group.The survival rates of skeletal muscle cells in various groups were measured by MTT assay;the cell apoptotic rates in various groups were detected by flow cytometry;the reactive oxygen species(ROS)levels were detected by DCFH-DA;Rhodamine 123 was used to detect the mitochondrial membrane potential;the expressions of Bax and Bcl-2 proteins were determined by Western blotting method.Results:Compared with normal control group,the survival rates of skeletal muscle cells in OGD group after 24 h OGD was significantly decreased(P<0.05);the apoptotic rate was markedly increased(P<0.01);the ROS level was increased(P<0.01);the mitochondrial membrane potential was decreased(P<0.01);the ratio of Bcl-2/Bax was significantly decreased(P<0.01).Compared with OGD group, the survival rates of skeletal muscle cells in different concentrations of EGF groups were increased and the apoptotic rates were decreased,especially in 10 and 20 μg·L-1EGF groups(P<0.05 or P<0.01);the ROS levels in skeletal muscle cells in different concentrations of EGF groups were decreased and the mitochondrial membrane potential were increased,especially in 10 and 20 μg·L-1EGF groups(P<0.05 or P<0.01);the Bcl-2/Bax ratios were significantly decreased in a concentration-dependent manner,especially in 10 and 20 μg·L-1EGF groups (P<0.05 or P<0.01).Conclusion:EGF can improve the skeletal muscle cell injury induced by OGD in a concentration-dependent manner via decreasing the ROS levels and protecting the cell mitochondrial function.

Objectvie: To observe the law of heat transfer in Zusanli during the process of moxibustion. Methods: Thetemperature change of Zusanli was detected by self-made high-precision temperature sensor while moxibustion on lowerextremities acupoints on six meridians of foot in healthy volunteers for fifteen minutes, the temperature change curve andtemperature change rate curve were drawn with Origin software. Results: Temperature change curve of Zusanli was thebiggest when moxibustion on acupoints in stomach meridian of foot yangming, especially for Shangjuxu acupoint.Whenthe acupoints near the level of Zusanli are moxibusted, the temperature change curve of Zusanli changes obviously.Temperature change rate curves of Zusanli were basically in coincidence when moxibustion on lower extremities acupoints on six meridians of foot, and it had a downward trend. Conclusion: Heat transfer between meridians andacupoints have propagated sensation along channel, regional characteristics, consistency of transmission rate, in theprosess of moxibustion.

Objective To investigate and analyze the clinical application of washed red blood cells in Quzhou in 2014-2015 years,and to provide evidence for the rational and effective use of washed red blood cells in clinical practice.Methods the amount of washed red blood cells,the distribution of diseases and the serological detection before transfusion were analyzed retrospectively in 8 hospitals of grade two and over in Quzhou during the past 2014-2015 years.Results the amount of washed red blood cells in 2015 increased by 39.07% compared with 2014,an increase of 24.4 times the amount of red blood cells increase,increase mainly in 3 hospitals;Diseases of the blood system,malignant tumors and chronic kidney disease is the main diagnosis of transfusion cases,accounted for 63.49%,19.05%,12.70%.There were different standards for the development of serological testing items before transfusion Conclusion the advantages of washed red blood ceils gradually recognized by clinicians,but also don't rely on experience,hospitals should pay attention to the comprehensive evaluation of clinical blood transfusion and standardize the serological detection of blood transfusion department,which is the key to improve the cost performance of this component.

Objective To study and analysis the effect of different doses of sodium phosphate solution in bowel preparation for colorectal treatment in children. Methods 100 patients who underwent colonoscopic polypectomy treated in our hospital from February 2015 to September 2016 were randomly divided into group A and group B, each group of 50 patients each. A group was treated with 2 bottles of cleansing liquid heating boiling water, B group were treated with 1 bottles of cleansing liquid heating boiling water. The therapeutic effects of the experimental group and the control group were compared and analyzed. Results The tolerance of group B was 78.0%, significantly better than that of group A(50.0%), and the difference was statistically significant (P<0.05). The incidence of adverse reactions in group B was 24.0%, significantly lower than that in group A (60.0%), and the difference was statistically significant (P<0.05). In group B, cleanliness was common in 1 patients and poor in cleanliness in 3 patients. In the A group, 1 patients had poor cleanliness and 2 patients had general cleanliness. There was no significant difference between the two groups. Conclusion 1 bottles of sodium phosphate in intestinal cleaning colorectal bowel preparation effect is good, low incidence rate of adverse reaction, greatly improve the intestinal cleanliness, with further clinical promotion and application significance.

Objective To discuss the effect of quick freezer equipment and quick freezing time on the preparation of cryoprecipitate.Methods The cryoprecipitate,already prepared,was placed into the MBF21 freezer and minus 30℃ SANYO refrigerator for 30-minute and 50-minute storage.The activity of coagulation factor VⅧ,and the content of fibrinogen in different equipment were detected by automatic coagulation analyzer,in order to make sure whether it met the quality requirements.Results The content of fibrinogen in the cryoprecipitates from both of the equipment after 30-minute storage met the requirement,with the qualification rate of 100%.As to the activity determination of coagulation FⅧ,low temperature refrigerator showed a qualification rate of 62.5%,which was significantly lower than that of quick freezer with a qualification rate of 97.5% (P<0.01).The cryoprecipitates from both of the equipment after 50-minute storage,with part of fibrin precipitation,had a qualification rate of fibrinogen content lower than 50% (P>0.05).The qualification rate of quick freezer and low temperature refrigerator was 35% and 12.5%,respectively,with significant difference.Conclusion Quick freezer can make the cryoprecipitate quick-frozen,which can ensure the quality.The coagulation factor VⅧ is unstable,whose activity decreased with the increase of temperature.We should try our best to shorten the time off the cold chain.

BACKGROUND:Hepatic fibrosis in chronic liver disease can be reversed. Studies have shown that Periplaneta americana extract has anti-fibrosis effect, and has protective effect on the experimental hepatic fibrosis rats. OBJECTIVE:To observe the effect of Periplaneta americana extract sticky sugar amino acid on hepatic fibrosis, and to primarily explore the mechanism of sticky sugar amino acid against hepatic fibrosis. METHODS:Rat models of immune hepatic fibrosis were induced by pig serum and intragastricaly administered 0.5, 0.25, 0.10 g/kg sticky sugar amino acid. Four indexes of hepatic fibrosis were detected by radioimmunoassay. Immunohistochemical staining wasused to measure transforming growth factor beta 1, tissue inhibitor of metaloproteinase protein expression intensity and positive cel rate, to determine the correlation of different concentrations of sticky sugar amino acid, transformation growth factorbeta 1 and tissue inhibitor of metaloproteinase. RESULTS AND CONCLUSION:(1) The Periplaneta americana extract sticky sugar amino acid reduced the levels of laminin, type III procolagen, type IV colagen and hyaluronidase (P 0.9). (3) Results confirmed that the Periplaneta americana extract sticky sugar amino acid can change the reversal of hepatic fibrosis. Its mechanism of action is associated with expression of transforming growth factor beta 1 and tissue inhibitor of metaloproteinase 1 inhibited by sticky sugar amino acids.

Objective To study the the effect of national free pre-pregnancy health check policy from the view of clinical labora-tory.Methods Couples who accepted pre-pregnancy check were recruited in the study.720 people were recruited.The data of pre-pregnancy health examination and related questionnaire were analyzed.Results In females,the IgG positive rates of rubella virus, cytomegalovirus and Toxoplasma were 80.34%,3.78% and 87.51%,respectively.IgM positive rates of Toxoplasma and cytomeg-alovirus were 0.73% and 1.24% respectively.17.98% females were found suffering from gynecological inflammation or ovarian cysts and other gynecological diseases.Syphilis test showed males positive rate was 0.83%,female positive rate was 0.59%.Ala-nine aminotransferase(ALT)test showed that male positive rate was 13.22%,female was 5.71%.For cognitive health examination surveys,92.33% males and 96.27% females had well known the importance of pre-pregnancy health examination.Conclusion Free pre-pregnancy health examination policy could provide a guarantee for the couple's family planning,and could reduce the incidence of malformation and neonatal congenital disease.

Objective To analyze the bloodstream infections caused by vancomycin‐resistant Enterococcus (VRE)in West China Hospital and examine the risk factors of such infections for best control of VRE infections .Methods Case‐control method was used to compare the bloodstream infections due to VRE with those caused by vancomycin‐susceptible Enterococcus(VSE)from August 2010 to August 2014 in West China Hospital .The data were compared by T‐test ,Chi‐square test (univariate analysis) and logistic regression analysis (multivariate analysis) using SPSS 19 .0 software .Results A total of 31 VRE strains were isolated from bloodstream infections in the study period ,including 5 (16 .1 % ) strains of vancomycin‐resistant Enterococcus faecalis and 26 (83 .9 % )strains of vancomycin‐resistant Enterococcus faecium .In the past four years ,the prevalence of vancomycin‐resistant E . f aecalis in bloodstream infections was 1 .5% ,1 .6% ,1 .8% ,and 1 .2% ;while the prevalence of vancomycin‐resistant E . f aecium in bloodstream infections was 3 .8% ,4 .4% ,5 .8% ,and 7 .1% .Such VRE bloodstream infection was mainly found in Intensive Care Unit (ICU)(13 ,41 .9% )and Department of Neurosurgery (4 ,12 .9% ) .More than 90% of the isolates were resistant to ampicillin and erythromycin .But less than 20% of the strains were resistant to quinupristin‐dalfopristin and linezolid . Univariate analysis indicated that prolonged hospital stay , ICU admission , venous catheter ,prior carbapenem exposure , prior vancomycin/norvancomycin exposure were associated with VRE bloodstream infections .Logistic regression analysis confirmed that venous catheter was an independent risk factor of VRE bloodstream infections .Conclusions Venous catheter is an independent risk factor for VRE bloodstream infections . Infection control measures should be strengthened to prevent the outbreak of VRE‐related bloodstream infections .

RESUMO

Objective To investigate the application of peroneal artery chain perforator flap in repairing skin and soft tissue defects of ankle and lower leg and the corresponding clinical effect.Methods From 2005 to 2010,19 cases of skin and soft tissue'defects of the ankle and lower leg were treated by using peroneal artery chain perforator flaps.The sites of peroneal artery branches were detected by using Doppler flowmeter before operation.Supplied by the chain form of terminal branches of the peroneal artery,the cutaneous fulcrum straightly nearest to the wound was selected as the rotation point to design the flap,with area ranging from 25 cm × 8 cm to 16 cm × 6 cm.Results The follow-up for 1.5- 42 months revealed that all cases of flap survived with satisfactory shape,except for one case suffered from distal skin flap necrosis.Conclusions The peroneal artery chain perforator flap with unsteady peduncle has no restrictions of peduncle sites and can be cut flexibly.It is a perfect method in repair of soft tissue defect of ankle and lower leg.

OBJECTIVE: Chengde is a city with many corneal blindness patients In north China. Lacking of corneas donation is the main reason which prevents corneal transplantation. Survey was made by questionnaire in Chengde to evaluate the current situation and the influential factors of corneal donation.METHODS: Survey was made in 3 200 Chengde residents aged 18 years or older, includes outpatients and inpatients of ophthalmology, some undergraduate students and people met accidentally in park, supermarket, station and centre for elders.48.6% are male and 51.4% are female. Self-made questionnaire includes general state, questions about cornea donation and factors influencing cornea donation.RESULTS: Among 3 200 questionnaires, 2 971 were valid. The effective rate was 92.84%. Over 50% people support donating cornea. More than 40% people intend to donate their corneas and support their relatives to donate. Among the factors for not intending to donate cornea, lacking knowledge of cornea donation was the main reason accounting for 42.81%, and worrying about the misusage of donating cornea without corresponding law became the second factor, which accounting for 21.07%. It has no influence on the consciousness of cornea donation by the difference of sex and location between city and countryside.Whereasfession and level of education indeed influence the consciousness of cornea donation, which of the people from 18 to 40 years old was greater than those of over 40, medical workers was greater than those from other fields, the people graduating from secondary specialized school or higher was greater than those graduating under secondary specialized school.CONCLUSION: People in Chengde have a positive attitude towards cornea donation. It is very necessary to enhance the education of cornea donation, establish an easy and smooth way for donation may promote cornea donating. Consummate legislation is also needed for cornea donation.

Objective:To clone mouse signal transducers and activators of transcription(STAT) 4 and STAT6 gene and construct a eukaryotic expression plasimid.Methods:The cDNA of mouse STAT4 and STAT6 gene amplified with RT-PCR from normal mouse spleen tissue were cloned into pMD19-T vector by T/A ligation.After double digested,STAT4/STAT6 cDNA fragment was subcloned into pIRES2-EGFP vector to construct a eukaryotic expression plasimid.Restriction endonucleases analysis and sequencing were used to comform the recombinant plasimid.Results:The full length STAT4/STAT6 cDNA was correctly inserted into eukaryotic expression plasimid,and its sequencing was consistent with reported sequence derived from Genebank.Conclusion:The successful construction of eukaryotic expression plasimid provides a basis for futher studies on the infection of STAT4/STAT6 interaction to the downstream cytokines.

RESUMO

[Objective] To establish a method for determination of serum tryptophan and kynurenine in pregnant women.[Method] RP-HPLC was applied to analysis.C18 column was used.The mobile phase was composed of methanol-0.001M KH2PO4(PH=4)(12∶88).The UV detection wave length was 245nm.[Results] The linear response ranged from 2.56~307.06?mol?L-1 for tryptophan, r=0.9999.The linear response ranged from 0.12~5.05?mol?L-1 for kynurenine, r=0.9997.The mean concentrations of serum tryptophan and kynurenine were 49.73?7.48?mol?L-1 and 0.74?0.33?mol?L-1 respectively in 30 pregnant women.[Conclusion] The method is sensitive and suitable for the determination.

RESUMO

Objective To investigate the myocardial protective effect of adenosine preconditioning against ischemia-reperfusion injury in patients who were undergone valve replacement during cardiac pulmonary bypass.Methods 42 patients scheduling for valve replacement operation were divided randomly into two groups:experiment group(n=23) and control group(n=19).Adenosine preconditioning were performed in experiment group before cardiac pulmonary bypass.To estimate the plasma level of cardiotroponin I (cTnI),creatine kinase isoenzyme (CK-MB ),radialis arterial blood was drown at following time points respectively: before operation,30 minutes before and after aortic clamp,1 hour and 3 hours after aortic opened.Meanwhile the rate of heart spontaneous rebeating,defibrillation,arrhythmia and cardiac inotropic agents used and its duration were recorded.Results The elevated range of plasm cTnI and CK-MB in experiment group were obviously lower than that in control group.In experiment group,the rate of heart spontaneous rebeating were higher,but the frequency of defibrillation,arrhythmia and the cardiac inotropic agents used and its duration were lower than those of control group.Conclusion Adenosine preconditioning has notable clinical protective effects on myocardial ischemia-reperfusion injury in patients undergone valve replacement during cardiac pulmonary bypass.
